Evaluation Forms

Here, you will find forms for evaluating fellows, faculty, and other program elements. Some elements may be password-protected.

Faculty evaluates…

    * fellow at end-of-rotation

    * fellow at mid-rotation (same form as above)

    * instant feedback to training director and/or fellow

Fellow evaluates…

    * herself/himself

    * supervisor (will be aggregated or anonymously presented so long as the supervisor cannot identify the trainee from context.)

    * teaching faculty (will be aggregated or anonymously presented so long as the faculty cannot identify the trainee from context.)

    * training program (to be completed annually, when progressing into the second year or on graduation.)

    * on-demand (non-scheduled) anonymous feedback to training director

360-degree Evaluations

These evaluations are from any discipline that may interact with the fellow. They can be instant or scheduled. They can be anonymous or named (such as to convey praise in writing.)

    * Simple feedback to fellow (optional to identify oneself)

    * Comprehensive evaluation (optional to identify oneself)

    * Patient evaluates working alliance (no protected health information is requested.)
